About this subject
On successful completion of this subject, students will be able to:
- Analyse the application of primary health care frameworks, considering the impact social determinants of health and health literacy may have on the patient and on the broader community.
- Justify the choice of examination and diagnostic approaches for determining diagnoses of patients presenting across a range of acuities.
- Develop evidence-based care plans and management pathways for patients with low acuity acute and chronic health conditions.

This subject focuses on clinical practice within the community and urgent care setting encompassing both primary and secondary health care needs. The subject integrates knowledge from structured clinical examination and diagnostics to inform diagnoses across complex chronic health conditions and those with common, acute or chronic primary health care needs. The subject explores evidence-based approaches to develop management plans and pathways of care in primary, secondary and community health care systems. The subject expands on patient-clinician communication, health literacy considerations, and the impact of social determinants of health while considering contemporary issues surrounding primary, secondary and community health care practice.
